Registration Information

• Registration cost for the course is $25.
• Payment must be received at the time of registration.
• Registration is on a first come first served basis.
• To register by phone or email contact: Laura Gayler at (801) 957-8620 or lgayler@utah.gov
• For registration by mail, click here to print an “Instructor Course Registration” form.
• Attendees must pre-register. Space limitations prevent us from allowing unregistered walk-ins the day of the class.

If you are planning on submitting your application or renewal at the end of the class you will need to bring all of your information to submit with your application. BCI can take your photo after you complete the course. The cost for the photo is $13.
