FBI Police Certificates
For complete information from the FBI about obtaining your FBI criminal history, please see the FBI's web page about obtaining your criminal history.
For FBI background check requests, you must submit:
- A complete set of fingerprints on FBI form FD-258. The "Reason Fingerprinted" block must contain "Personal Records Check."
- A money order or certified check in the amount of $18.00, payable to the U.S. Treasury. Payment may also be made by credit card (refer to the FBI link above.)
- A letter stating why the background check is needed.

The applicant cards and fingerprinting services are available at the Utah Bureau of Criminal Identification (3888 West 5400 South, Salt Lake City), Monday thru Friday from 8:00 am to 5:00 pm at a cost of $15.00 per person. We are closed weekends and state holidays.
You can also check with local police departments, motor vehicle offices, or private industries for these services. Do not contact the FBI for fingerprinting.
