This form can be filled out and submitted in one of two different ways:

When you have entered all the necessary information, you have the choice of emailing or mailing the report to BCI. To email it, save the form, then attach it to an email addressed to dellamiller@utah.gov.
To mail the report, print the completed form and mail it to:
