NOTE: As of January 1, 2010 there are new requirements for obtaining a duplicate Utah driver license due to changes in legistlation.  Please click here for more information on what will be required to obtain a duplicate license.

 

If you have misplaced your driver license, you will need to visit a licensing office to obtain a duplicate license.

If your license was a Utah driver license you will need to:

• Complete an application
• Present 2 forms of positive identification
• Provide proof of Utah residence
• Pay the applicable fee
• Take a new photo

Note ** if you have forgotten your social security number, you must bring your social security card with you.  We cannot provide you with your number.

If your license was an out-of-state driver license you will need to visit our "I am new to Utah and need a Driver License page."

After all requirements for a license have been successfully completed, you will receive a temporary photo license on the same day. The completed license will then be mailed to you in approximately three weeks.
