I am 18 and under and have lost my Utah Driver License

NOTE: As of January 1, 2010 there are new requirements for obtaining a duplicate Utah driver license due to changes in legistlation.  Please click here for more information on what will be required to obtain a duplicate license.

If you have misplaced your Driver License, you will need to visit a licensing office to obtain a duplicate license. Because your parent or legal guardian has already signed for financial responsibility, they will not have to sign again, however, you will need to complete an application and bring the following documents with you:

• Present 2 forms of positive identification
• Provide proof of Utah residence
• Pay the applicable fee
• Take a new photo

Note ** if you have forgotten your social security number, you must bring your social security card with you.  We cannot provide you with your number.

 

After all requirements for a license have been successfully completed, you will receive a temporary photo license on the same day. The completed license will then be mailed to you in approximately three weeks.
