I am 18 and under and have lost my Utah Driver License

NOTE: As of January 1, 2010 there are new requirements for obtaining a duplicate Utah driver license due to changes in legistlation.  Please click here for more information on what will be required to obtain a duplicate license.

If you have misplaced your Utah driver license, you will need to visit a licensing office to obtain a duplicate license. Because your parent or legal guardian has already signed for financial responsibility, they will not have to sign again, however, you will need to complete the following:

• Take a new photo;
• Complete an application (fill out our online application and print ahead of time);
• Present documents required;
• Pay the applicable fee;

After all requirements for a license have been successfully completed, you will receive a temporary photo license on the same day. The completed license will then be mailed to you in approximately three weeks.
